Health & Wellness Coordinator

Job description


This position performs the Health and Wellness Coordination for the Clients in their assigned homes. The primary purpose is to assist the nursing staff with all medical aspects including appointments, training and mentoring the home staff.

Specific Areas of Responsibilities:

  • Assure that medical appointments, medications, diets and risk plans are run such that each person’s quality of life is maximized.

  • Schedules appointments, monitors and makes sure adaptive equipment and other materials in homes are in good working order.

  • Serve as liaison between the home and community, day program, parents, or other involved agencies or individuals.

  • Participates in ISP/PCP related activities.

  • Mentors staff in medication administration and is responsible for medical appointments

  • Mentors and trains staff in job duties associated with health and wellness activities and procedures

  • Participate in annual case conferences and quarterly meetings

  • Be part of the team that ensures the home is in compliance with the State Dept. of Health regulations.

  • Know and adhere to all Corvilla’s policies and procedures

  • Attend community meetings, conferences and workshops as deemed appropriate by supervisor

  • Responsible for HWC weekly log and notes

  • Monitor the cleanliness of the homes for health and safety

  • Work along with the RI during active treatment hours weekly, to mentor, train, supervise and be good role model

  • Communicate all necessary items to appropriate Administrative Staff

  • All other responsibilities as assigned

Position Requirements:

  • High School Diploma/GED

  • Previous experience working in a group home environment with medical appointments

  • Previous experience training and mentoring staff preferred.

  • All positions at Corvilla, Inc. require that employees are free from all felonies, have a valid driver’s license and employee must be insurable by our insurance carrier.

  • Must possess quality domestic skills, good written and verbal communication skills.

  • Ability to lift up to 50 lbs

  • Must be even tempered and patient!

Job Type: Full-time

Salary: $18.00 per hour

Schedule: 40 hours per week

  • 8 hour shift

  • Monday to Friday

  • On call


  • High school or equivalent
